The Mindset Reframe I Use With My Clients Who Have IVF Anxiety

It's 3am. You're staring at the ceiling. Your brain is wide awake, running through a scan that hasn't happened yet, a phone call that hasn't come, a conversation with a friend who hasn't even fallen pregnant.
If you know this hour, you already know what IVF anxiety at night feels like. One small question turns into fifteen minutes of a full mental movie. What if my scan tomorrow shows zero follicles? What if this cycle is cancelled? Before you've even had your coffee, you've imagined pulling yourself together to go back to the office, telling your family it didn't work, planning the next cycle, looking into donor eggs.
All from one little question.
I want to be really clear about something before we go any further. You are not broken for spiralling like this. You are not losing your mind, even though it feels that way. I have clients who have never struggled with anxiety a day in their life, and the second they start IVF, it shows up and takes over. There's a reason for that, and once you understand it, the spiral starts to lose some of its grip.
Why IVF Makes Even Calm People Anxious
Your brain hates uncertainty. It wants a plan, a timeline, a guarantee. IVF gives you none of that. It gives you unknown after unknown after unknown, and your brain does not sit quietly with unknowns. It fills the gaps. It runs the worst case scenario on a loop, trying to prepare you, trying to protect you.
Here's the problem. Your nervous system can't tell the difference between something that's actually happening and something you're imagining. So when you run through that worst case scenario in vivid detail, your body responds as if it's real. Tension. Racing heart. That horrible pit in your stomach. All from a scenario that hasn't happened and might never happen.
While your brain thinks it's doing you a favour, it's actually adding a second layer of suffering on top of a situation that's already hard enough. You're not just living through today's appointment, today's injection, today's wait. You're also living through every terrible outcome your brain can invent, on repeat, before any of it is real.
That's the cost of worst case scenario thinking. It doesn't protect you from pain. It just makes sure you feel it twice.
The Reframe That Changed Everything For Me
I heard something on a podcast a while back that stopped me completely in my tracks. The woman being interviewed was telling a story about sitting in a hospital while her mum was critically ill. She was terrified her mum was going to die, and she couldn't stop spiralling. How would she cope? How would she survive without her? How would she manage it all, logistically and emotionally?
A friend said something to her that changed the way she thought about fear for good. She said the version of her who handles that moment, if or when it happens, doesn't exist yet. That version of her will be born into existence at the exact moment she's needed, and she'll have more strength, more wisdom and more context than the person sitting in that hospital chair right now.
I must have replayed that line ten times. It's one of those ideas that sounds simple on the surface but completely reorganises how you think once it lands.
Bring that back to IVF and it changes the whole conversation. What if my body doesn't respond to the meds? What if this transfer fails? What if everyone around me falls pregnant except me? These are all problems you're trying to solve right now, in advance, before you even need to. But you don't need to know today how you'd cope with any of that, because the version of you who copes with it doesn't exist yet either.
She only shows up on the other side of that moment, if it even happens. And here's the part that matters most. She always does.
The Proof Is Already In Your Own Story
If you want evidence, look at what you've already survived. The call that none of your embryos came back normal from PGT testing. The cycle that didn't work. The second one. The third. Everything you've made it through up until this exact point, that's not nothing.
At the time, you probably thought, I don't know how I'll get through this. But you did. Maybe a bit more weathered, a bit more beaten up, but still here, still doing this. That's proof future you shows up when it counts, every single time.
So the current version of you doesn't need to have all the answers for a problem that hasn't happened yet. Future you will have the skills she needs, because every single day she's learning more. She'll figure it out. She always does.
The Mantra For When You Start Spiralling
Next time your brain starts running ten steps ahead, this is what I want you to come back to.
Today me doesn't need to solve that yet. Future me has my back. She'll figure it out when she needs to.
That's the whole shift. It's not about pretending bad things won't happen, and it's definitely not blind optimism. It's about giving yourself permission to stop suffering twice. Once in your imagination, running through scenarios that may never exist, and again if it actually happens. You are already holding enough without adding a second round of pain on top.
Why The Mindset Shift Isn't The Whole Answer
I want to be straight with you here, because that's how I roll. A mindset shift and a mantra will genuinely change how you experience the spiral. But a mantra alone isn't the whole picture. What you also need is to be building real coping skills in current you, so that when future you shows up, she's actually got something to draw on.
The hard moments in IVF can build resilience, but they can also strip something away from you if you're moving through them without support. Learning how to cope with IVF anxiety isn't just about having the right words for the 3am spiral. It's about building the steadiness to handle the moments that are real and hard today, and the ones that might still be ahead. Because sometimes the worst case scenario does happen, and I'm not going to pretend otherwise.
The goal isn't hoping you'll cope if that call comes, or the test is negative, or your friend tells you she's pregnant. The goal is knowing you will, because you've already built that trust in yourself before you needed it.
